Mounjaro Dosage Guide UK: 2.5mg to 15mg Dose Schedule
Mounjaro® (tirzepatide) is usually started at 2.5mg once weekly and increased gradually if needed and tolerated. Dose changes should always be guided by a prescriber.

Quick Answer: What Is the Usual Mounjaro Dosage Schedule?
The usual Mounjaro starting dose is 2.5mg once weekly for 4 weeks. After this, the dose is usually increased to 5mg once weekly. If further dose increases are needed and tolerated, the dose may be increased in 2.5mg steps at intervals of at least 4 weeks.
The available dose strengths are 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg, and 15mg once weekly. The maximum licensed dose is 15mg once weekly.
Important: You do not have to increase your dose every month. If your current dose is working well or side effects are troublesome, your prescriber may advise staying on the same dose for longer.

Why Does Mounjaro Start at 2.5mg?
Mounjaro is started at 2.5mg once weekly to help your body adjust. This gradual approach is designed to reduce the chance of side effects such as nausea, vomiting, diarrhoea, constipation, indigestion, and reduced appetite.
The 2.5mg dose is an initiation dose. Some people notice appetite changes during the first month, but the main purpose is tolerability rather than rushing weight loss.
When Do You Move from 2.5mg to 5mg?
After 4 weeks on 2.5mg, the usual next step is 5mg once weekly. Your prescriber may review how you are getting on before increasing the dose, especially if you have had side effects or concerns.
If treatment is tolerated, 5mg may become an ongoing dose for some patients. A higher dose is not always needed if you are losing weight steadily, appetite is controlled, and side effects are manageable.
Do You Have to Increase Your Mounjaro Dose Every Month?
No. Dose escalation should be individualised. Although the dose can be increased in 2.5mg steps after at least 4 weeks on the current dose, that does not mean every patient must increase every month.
Your prescriber may advise staying on the same dose if:
- you are losing weight at a safe and steady rate
- your appetite is well controlled
- you are experiencing side effects
- you are close to your target or maintenance phase
- a higher dose may not be clinically necessary
Higher is not always better
The best Mounjaro dose is the dose that is effective, safe, and tolerated for you. Increasing too quickly may increase side effects without improving long-term results.
What Are the Maintenance Doses of Mounjaro?
For weight management, ongoing doses are selected based on response and tolerability. Some patients do well on 5mg, while others may need 10mg, 12.5mg, or 15mg under prescriber supervision.
Maintenance does not always mean staying on the highest dose. It means finding a dose that supports weight control, side effect management, and long-term treatment goals.
Side Effects During Dose Escalation
Side effects can occur at any dose but are often more noticeable when starting treatment or increasing the dose. Most common side effects are gastrointestinal.

What If You Miss a Mounjaro Dose?
If you miss a Mounjaro dose, the usual advice is:

Do not take two doses close together to make up for a missed dose. There should be at least 3 days between Mounjaro doses.
What If You Stop Mounjaro for Several Weeks?
If you have had a break from Mounjaro, do not restart at your previous dose without clinical advice. After a treatment gap, your body may no longer tolerate the same dose, and restarting too high may increase side effects.
Your prescriber may recommend restarting at a lower dose depending on how long you have been off treatment, your previous dose, your side effect history, and your current weight and BMI.

Can You Move Down a Mounjaro Dose?
Yes, in some cases a prescriber may advise moving down a dose. This may be appropriate if you are having side effects, appetite suppression is too strong, your weight loss is faster than intended, or you are entering a maintenance phase.
Dose reduction should be discussed with your prescriber, especially if you have diabetes, are taking other medicines, or have ongoing symptoms.
