While Mounjaro is effective, it isn’t for everyone. You should not use Mounjaro if any of the following apply:

  • Allergy: You have had an allergic reaction to tirzepatide or any ingredients in the formulation. Signs of a serious allergy could include rash, swelling, or difficulty breathing – if you’ve experienced these with similar injections, avoid Mounjaro
  • Certain Medical Histories: Mounjaro is not recommended if you have a personal or family history of medullary thyroid carcinoma or MEN2 (multiple endocrine neoplasia type 2). (These thyroid conditions are rare, but GLP-1/GIP medications carry a precaution because they caused thyroid tumors in animal studies. As a safety measure, you shouldn’t take Mounjaro if these conditions apply to you.) Also, if you have a history of pancreatitis (inflammation of the pancreas) or severe gastrointestinal disease, Mounjaro may be contraindicated due to the risk of exacerbating these issues
  • Pregnancy & Breastfeeding: As noted, do not use if you are pregnant, trying to become pregnant, or currently breastfeeding, weight loss medications are not used during pregnancy, and it’s unknown if tirzepatide passes into breast milk.
  • Age: Under 18 years old. Mounjaro is an adult-only treatment.

Safety Considerations: Mounjaro was approved for weight loss relatively recently (the MHRA approved it in the UK in November 2023 for obesity treatment). Because it’s new, it has a ▼ (black triangle) symbol, meaning it’s under additional monitoring by regulators. If you have other health conditions (like kidney problems, liver issues, or diabetic eye disease/retinopathy), our prescriber will carefully evaluate if Mounjaro is appropriate. In some cases, an alternative weight loss treatment or a lower starting dose might be recommended. We take a cautious approach to ensure your safety.

Dosage Schedule: Mounjaro treatment follows a gradual dose-escalation schedule. This means you start at a low dose and increase over time, allowing your body to adjust and helping to minimize side effects. The typical Mounjaro dosage progression is:

  • Start with 2.5 mg once weekly for 4 weeks- this is the initial dose for weeks 1–4.
  • Increase to 5 mg once weekly for at least 4 weeks (weeks 5–8).
  • If needed, 7.5 mg weekly for 4 weeks, then 10 mg, then 12.5 mg, and up to 15 mg weekly– each increment should be done after at least 4 weeks on the previous dose and only if the previous dose is well-tolerated and more weight loss is desired. The maximum dose is 15 mg weekly

Not everyone will go up to the highest dose. Your prescriber will typically assess your progress and tolerability at each stage. Many patients achieve good results at 5 mg or 10 mg and stay at that as their maintenance dose. Higher doses can produce additional weight loss but also carry a higher chance of side effects.

You and our clinician will decide the optimal maintenance dose based on your weight loss progress and how you feel on the medication. Never increase your dose sooner than directed (usually no faster than every 4 weeks) and do not exceed 15 mg per week.

 

Injection Instructions: Mounjaro comes in an easy-to-use autoinjector pen for subcutaneous (under the skin) injection. Unlike a traditional syringe, the pen has a tiny needle and is designed for patient self-administration. You’ll inject Mounjaro once a week on the same day each week. Here are the basics:

  1. Prep the Pen: Each pen is single-use (one weekly dose) and comes pre-filled. Take a pen out of the fridge about 30 minutes before using it (injecting cold solution can sting; room temperature is more comfortable). Check the pen’s label to make sure it’s the correct dose and not expired. Attach a new needle to the pen as instructed.
  2. Choose Injection Site: The recommended injection areas are the abdomen (stomach) or thigh, or the back of the upper arm drugs.com (if someone else is giving you the injection). Most people find the stomach or thigh easiest for self-injection. Clean the skin with an alcohol swab. Rotate your injection site each week – for example, alternate between left/right abdomen or different thighs – to avoid irritation do not inject within 5 cm of the navel when using the abdomen, to avoid sensitive areas.)
  3. Injecting: Insert the needle into the skin (pinch up a fold of skin if needed). Activate the pen by pressing the injection button or plunger as described in the pen’s instructions. Mounjaro pens will deliver the dose automatically; you might hear a click. Keep the pen in place for about 5-10 seconds to ensure the full dose is delivered. |Then remove it and dispose of the needle safely in a sharps bin. Dial the dose knob to “0” (if applicable) to confirm the dose is complete. Each pen is single-dose only – after injection, dispose of the used pen properly (we include a sharps container with your order for safe disposal).
  4. After Injection: You can gently press on the site with a cotton ball if there’s a drop of blood, but do not rub hard. Many people find the injections are relatively painless due to the very fine needle (smaller than those used for typical vaccines). If you’re anxious about self-injecting, our pharmacists can provide tips or a demonstration video link. With a bit of practice, most patients become comfortable with the weekly routine.

Timing: Take Mounjaro once a week, on the same day each week (for example, every Monday morning). It can be taken any time of day, with or without food. Pick a day that works best for your schedule and set a reminder so you don’t forget. Consistency is key for the medication to work properly. If you need to change your injection day, make sure there’s at least 3 days (72 hours) gap from your last dose before starting the new schedule this prevents overlapping doses).

 

Missed Dose: If you miss your weekly dose, take it as soon as you remember, within 4 days (96 hours) of the missed dose. If more than 4 days have passed, skip the missed dose and just take your next dose at the regular scheduled time. Do not double up or take two doses in a week to make up for a missed one. If you miss doses frequently or have trouble remembering, speak with us for advice – we can help with reminders or discuss whether Mounjaro is still appropriate.

 

Mounjaro pens are available in different strengths and color-coded labels. You’ll typically start on the 2.5 mg pen (weeks 1–4) and then move to higher doses like 5 mg (weeks 5–8) as needed. The pen device is designed for simple, once-weekly self-injection.

 

Treatment Duration: How long you’ll stay on Mounjaro depends on your weight loss goals and how you respond. Typically, a treatment trial of about 3–6 months will indicate how well it’s working for you. Many patients will use it for several months to over a year to achieve significant weight loss and then may continue long-term at a maintenance dose if needed for weight maintenance (similar to how other obesity treatments are used). If you reach a healthy weight or find that Mounjaro is no longer needed, the prescriber may advise tapering off. Conversely, if you are not losing sufficient weight after 3-4 months on the maximum tolerated dose, the treatment might be re-evaluated. Our team will provide guidance based on your progress. Remember, there is no fixed “course” for weight loss injections – it’s individualized. Some people use them short-term, others longer, always with the goal to improve health.

 

After Stopping Mounjaro: If you decide to stop Mounjaro (or run out of pens and don’t continue), be aware that your appetite will likely return to normal. It is possible to regain weight if the healthy lifestyle changes are not maintained. We strongly encourage continuing with your improved diet and exercise routine even after stopping the medication. Think of Mounjaro as a tool that helps you establish those good habits – you’ll want to keep those habits going to preserve your results. Any side effects you experienced on Mounjaro should resolve within a few weeks after stopping

Like all medications, Mounjaro can cause side effects. Understanding what to expect can help you manage them and know when to seek help. The good news is that, for most people, side effects are mild-to-moderate and improve over time as your body adjusts.

During clinical trials and real-world use, the following side effects have been observed:

  • Very Common Side Effects: Gastrointestinal (GI) symptoms are the most frequent. These include nausea (feeling sick), vomiting, diarrhea, and constipation

Up to a third of patients might experience nausea, especially when starting or increasing the dose. These effects are usually temporary and often ease as you stay on the same dose. Eating smaller, bland meals and staying hydrated can help.

  • Common Side Effects: These may affect some patients and are generally mild. They include indigestion, stomach pain or cramping, bloating, excess gas (flatulence), burping, or heartburn.

Some people report decreased appetite (which is part of how the drug works) or a change in taste. Other common effects can be dizziness, fatigue (feeling tired), or injection site reactions (like redness or itching where you injected).

A small number of patients have noted hair thinning or loss during rapid weight loss – it’s unclear if this is due to the drug or the weight loss process, but hair typically grows back as your weight stabilizes.

  • Low Blood Sugar (if Diabetic): If you have diabetes and take insulin or certain tablets, Mounjaro can increase the risk of hypoglycemia (low blood sugar) when combined. We will adjust your other diabetes medications if needed. Symptoms of low sugar include shakiness, sweating, and confusion – treat with a sugary snack or drink and contact a doctor. (If you’re not on other blood-sugar-lowering meds, Mounjaro by itself does not usually cause hypoglycemia.)
  • Uncommon/Rare Side Effects: Gallstones can occur with substantial weight loss or with GLP-1 drugs – report any sudden severe right-side abdominal pain or yellowing of skin/eyes

Increased heart rate has been noticed in some cases

Pancreatitis (inflamed pancreas) is a rare but serious risk; if you have severe persistent abdominal pain (with or without vomiting), seek medical help immediately.

Allergic reactions are very rare but possible – if you experience swelling of the face/tongue, or difficulty breathing (signs of anaphylaxis), call 999 for emergency help.

Most side effects tend to happen after a dose increase and then improve. To minimize stomach upset, follow the dosing schedule – don’t jump to a higher dose too quickly. If you do get nauseous, try eating smaller meals and avoid rich or greasy foods. Ginger tea or plain crackers can help with nausea. For constipation, ensure you drink plenty of water and consider a fibre supplement or stool softener if needed; for diarrhoea, stay hydrated with electrolytes. Over-the-counter remedies (like antacids for heartburn or paracetamol for a headache) can be used if appropriate – ask our pharmacist if you’re unsure.

  • Active Ingredient: Mounjaro contains tirzepatide, which activates two hormones (GLP-1 and GIP). Wegovy contains semaglutide, which activates only GLP-1 receptors. Ozempic is actually the same drug (semaglutide) as Wegovy, but Ozempic is the lower-dose version branded for diabetes treatment. Essentially, Wegovy and Ozempic are both semaglutide – Wegovy is used for weight loss at a 2.4 mg/week dose, while Ozempic is typically up to 1.0 mg/week for diabetes (though some doctors use Ozempic off-label for weight loss if Wegovy isn’t available).

  • Dosage Schedule: Both are weekly injections. Wegovy (semaglutide) has a fixed dosing schedule from 0.25 mg up to 2.4 mg over 16-20 weeks. Mounjaro’s dosing (as described above) goes from 2.5 mg up to potentially 15 mg, usually increasing every 4 weeks. So Mounjaro offers more gradations in dose. Some patients who struggle with semaglutide side effects might benefit from Mounjaro’s more gradual titration – or vice versa.
  • Approval and Availability: Wegovy (semaglutide) was approved earlier specifically for obesity and has been in use longer for weight loss (approved in 2021 in the US, 2023 in the UK). Mounjaro was approved for weight loss in late 2023 in the UK.

Wegovy is made by Novo Nordisk, and Mounjaro by Eli Lilly. Both are prescription-only and not yet available on the NHS for general weight management (see below). Saxenda requires daily injections and typically results in ~5-10% weight loss, so less than Wegovy/Mounjaro, but it’s an option if weekly isn’t suitable. 

 

Orlistat is a pill (taken with meals) that can help lose about 5-10% weight, but can cause gastrointestinal side effects (oily stools) if you eat fatty foods. Mysimba is a pill that works on the brain’s appetite center; some patients lose around 8% body weight with it.

 

We also consider non-medication routes: for those who prefer not to take medicines, structured programs like Weight Watchers or low-calorie diet plans, or even bariatric surgery for those with very high BMI, are alternatives. But for most patients looking at Mounjaro, the reasonable alternatives will be Wegovy or Saxenda if they want an injectable, or possibly an oral med if injections are a no-go.

 

If you have to stop Mounjaro due to side effects, sometimes we try a different GLP-1 (like semaglutide) which might be better tolerated – or a lower dose for longer.
